                         Student Information Form. PDF
                  The student information form must be completed by all
                  student teaching applicants and submitted to Linda
                  Wulf, Director of the Multimedia Center by the second
                  week in January of your junior year to schedule student
                  teaching during the following academic year. The form
                  should be downloaded, completed, printed out, signed,
                  and with the appropriate, completed application form
                  submitted to Mrs. Wulf.
                  Select and complete the appropriate application form
                  and submit it with your student information form.
          Portfolio Artifacts. PDF
                  Your pre-student teaching portfolio must be evaluated by
                  three professional individuals as indicated on the first page of
                  the assessment form. Download and print the form and submit
                  it along with your portfolio to the three individuals you have
                  chosen to perform the evaluation.  The completed evaluation
                  form must be submitted to Linda Wulf, Director of the multimedia
                  Center no later than March 15 of the academic year prior to your
                  scheduled student teaching experience. The portfolio artifacts
                  document provides suggestions for the inclusions of items in each
                  section of your portfolio.
                 This document identifies policies and procedures
                 regarding student teaching at UE, and also provides
                 information concerning your rights and responsibilities
                 and the rights and responsibilities of your University
                 Supervisor and Cooperating Teacher during your
                 student teaching experience.
